Currently we have limited availability. If you are a parent referring your teen to our adolescent program, complete the form using your child’s contact information and include your contact information in the last section. Individuals who are age 18 or older must complete their own form, with the exception of legal guardians.
Please note that we do not have an Intensive Outpatient Program (IOP) and our center is not equipped to respond to a crisis.
